What are the chief reasons you believe, or have seen, that EMR implementations fail?

simple: 99% are NOT designed around the practitioner delivering the care. we do not think in templates so don't try to make us fill them in. IF IT TAKES MORE TIME TO COMPLETE AN NOT IN AN EMR than to write it on paper with check off boxes, then it is too expensive EVEN IF IT IS FREE!

It is kinda what I have been researching over the past couple of years and here is my answer:

Now working to help solo, small to medium practices implement as painlessly as possible.

The biggest problem that I have seen comes down to Clnical function and need being put aside for technology limitation.  We got way to excited about things that we could do just to find that we had to change the whole practice to fit the EMR instead of the other way around.

I do agree with the post above, the biggest cause of the emr implementation failure is the mismatch between the application and the practice. The other reasons would include lack luster training and motivation to use the new system. If people find slight modifications on a social networking site a nuisance, new work flow would definitely be a big issue. EMR selection and implementation is a huge step, its better to have a selection and implementation(step) plan, to ensure that you find the perfect fit for your practice and make the implementation process simple for those concerned. A good EMR company can help you achieve the latter to some extent and also provide you with customizable templates. I work as a HIT research analyst and having a good client service definitely goes a long way, i would recommend a company like curemd. They are one of the oldest emr companies they have a good client base and their business thrives on their client service department. here is their emr page: http://www.curemd.com/emr.htm

Those who have implemented EMR, may I ask who were the people involved in the actual design of your program for your office?
